- newly diagnosed cancer
- recent heart attack
- joint replacement
- other acute trauma or surgery

The Continuity of Care form must be returned to the address on the form before the 1/1/2022 effective date of your coverage. No forms will be accepted after that date. To ensure no interruption in service, please submit the request form by 12/1/2021.
